Referrals

We encourage faculty and staff to refer students, in need, to a student counselor.  Please use the attached referral form.  Then arrange a meeting with and submit a hard copy of the form to the campus counselor.  DO NOT send the form via email or fill it out online.

We strongly recommend you speak to the student first regarding your concern, then inform your Asst Chair or Chair in case the matter can be addressed by the academic department.
